Insulin Resistance and Weight Gain: Signs, Tests and Treatment

Quick answer: Insulin resistance means your cells do not respond properly to insulin, so your body makes more of it. High insulin levels make it easier to store fat, especially around the waist, and harder to lose it. It is very common in South Asians and often comes before type 2 diabetes. Blood tests such as fasting glucose and HbA1c can detect it, and losing even 5 to 7 percent of your weight, eating fewer refined carbohydrates and staying active can improve it significantly.

What is insulin resistance?

After you eat, your blood sugar rises and your pancreas releases insulin, which moves sugar into your cells for energy. With insulin resistance, the cells stop responding well, so the pancreas works harder and produces more insulin to keep blood sugar normal.

For a while, this works. But high insulin levels encourage fat storage and make it harder to burn fat. Over time the pancreas can struggle to keep up, blood sugar starts to rise, and prediabetes and then type 2 diabetes can develop.

Why is insulin resistance so common in Pakistan?

  • Genetics: South Asians are more likely to develop insulin resistance and to store fat around the abdomen and organs, even at a normal-looking weight.
  • Diet: meals high in white rice, naan, sugar, sweet chai and fried food cause large blood sugar spikes.
  • Less movement: desk jobs, long commutes and hot weather reduce daily activity.
  • Family history: according to the International Diabetes Federation, Pakistan has one of the largest numbers of adults living with diabetes in the world.

This is also why doctors use lower BMI cut-offs for South Asian adults.

What are the signs of insulin resistance?

Insulin resistance often has no obvious symptoms, but common signs include:

  • weight gain mainly around the waist, and difficulty losing it
  • strong cravings for sweet or starchy food
  • feeling tired or sleepy after meals
  • dark, velvety patches of skin on the neck, underarms or elbows
  • skin tags
  • irregular periods or PCOS in women
  • high blood pressure, high triglycerides or a fatty liver on ultrasound

Which blood tests check for insulin resistance?

A doctor may recommend:

Test What it shows
Fasting blood glucose Your blood sugar after an overnight fast
HbA1c Your average blood sugar over the past two to three months
Fasting insulin How much insulin your body needs to keep blood sugar normal
Lipid profile Cholesterol and triglycerides, often raised with insulin resistance
Liver function tests Signs of fatty liver

Prediabetes is usually diagnosed from fasting glucose or HbA1c results. Different guidelines use slightly different cut-offs, so your doctor will explain what your numbers mean. At AQ Wellness, every weight loss package includes comprehensive blood tests reported by our pathology partners.

Can insulin resistance be reversed?

In many people, yes, especially when it is caught early. The landmark Diabetes Prevention Program found that modest weight loss and regular activity reduced the risk of developing type 2 diabetes by 58 percent in people with prediabetes.

Eat to keep blood sugar steady

  • Fill half your plate with vegetables and add protein to every meal.
  • Choose whole-wheat roti and smaller rice portions.
  • Swap sugary drinks and sweet chai for water and unsweetened tea.
  • Eat vegetables and protein first, and carbohydrates last.

Move after meals

A 10 to 15 minute walk after lunch and dinner helps your muscles use glucose. Strength exercises two or three times a week build muscle, which improves insulin sensitivity.

Sleep and stress

Poor sleep and chronic stress both worsen insulin resistance. Aim for regular sleep and find simple ways to switch off.

Medical treatment

Some people need medication alongside lifestyle changes. Depending on your results and health, a doctor may discuss metformin or GLP-1 based medicines, prescribed only when clinically appropriate. Read our guide to weight loss injections in Pakistan.

Frequently asked questions

Does insulin resistance cause weight gain?

Yes. High insulin levels encourage fat storage, particularly around the abdomen, and increase hunger and cravings, which makes weight gain easier and weight loss harder.

How do I know if I have insulin resistance?

You cannot be sure from symptoms alone. Blood tests such as fasting glucose, HbA1c and sometimes fasting insulin, together with your waist size and medical history, give a clear picture.

Is insulin resistance the same as diabetes?

No. Insulin resistance can exist for years before blood sugar rises enough to be called prediabetes or diabetes. Treating it early can prevent or delay diabetes.

How long does it take to improve insulin resistance?

Many people see improvements in their blood tests within a few months of consistent changes. Individual results may vary.
